SD shield 3.0 do not save well data, here is a program to show, HELP needed plea

i tryied with  uno and  mega,
     i did:

----5v sd shield (more error 3.3v ,   )
---mega pin 10, 53 , pin 4,  of them work same
---more delays
---double close.
---change serial speed (all....)
--- take off jumpers sd shield
--- format 16 , 32 bits
--- change 4gb others


data still lossing, data , work fine few rows, not
sometimes "noise" data

i can not confirm  problem, don't think problem in software attached. please post code using code tags.

i have same itead v 3.0 shield 4gb card running in top standard sized receptor. cs = 4. mounted on uno running usb, , shield set 5v. using ide v 1.6.0.

i didn't use sensors, use jumper wires connect a0 5v, a1 3.3v , a2 ground. readings accurate , formatting never broken.

i think problem still possibly low voltage shield. can confirm 5v?

even though run code without problems, suggest eliminate concatenated string objects code, , work data , character arrays. strings can cause memory leaks , create problems.

are glitches related trying remove , reinsert card?
